Based on the live recurring series to be held quarterly to address hot topics related to interprofessional team performance and patient care. Videos and resources are shared to improve teamwork across the spectrum of licensed healthcare professionals.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Review the (IRF) Inpatient Rehabilitation Form.
  2. Integrate patient care with team planning.
  3. Explain what social determinants of health care are and why they are part of ethical patient-centered care.
  4. Demonstrate essential skills for exploring and addressing social determinants as a team.
  5. Describe traditional team members' roles and training.
  6. Describe and discuss the flexible application of roles within the healthcare team.
  7. Discuss leadership as a team role (guild vs. team).
  8. Practice the skill of articulating your role as a member of the healthcare team.
  9. To discuss aspects of teamwork in any setting (even non-clinical) that impact team functioning and outcomes
  10. To describe team processes/infrastructure that allows for better-coordinated care such as team huddles and/or hand-offs
